Your hard drive is physically dying. You need to back it up immediately (if you haven't already) and replace the drive.
Apple can do this for you, and it may be free (depending on Apple care status) or cost you money, or a third party Apple repair place can do it (also will cost you).
You can also do it yourself if, but this requires confidence with computer repair - it is not trivial and you could do more damage if you don't know what you're doing, even if you follow a guide.
