didn't you find this theme on the "Post your Jailbroken home screens" (don't know exact title). Why don't you ask the creator to give you it? He posted the picture of the homescreen on that thread.

The dock was found but it's not iPhone 4 ready yet (it was also posted in the thread). I'm fairly sure you could recreate the wallpaper easy enough. As for the icons, they seem to be modified suave icons?

What about the weather and date/day? Any clues there?

It's Suave, and it looks like he has 4x4 dense along with it.

Indeed, he appears to be using Iconoclasm with 4x3 dense.

Few things, Suave HD requires alot of work to get the icons right. Resizing mainly to get rid of the black borders. Bare in mind also that he is using normal Suave as he has a 3G/S, which has alot more icons.

We've been over it:

The theme is Suave, judging from the icons, he has to have a 2G, 3G, or 3GS (because some of the icons havent been ported to the iPhone 4 yet).

The icons arrangement is the use of Iconoclasm, a paid app from Cydia to arrange the icons like that.

Thats a wallpaper. Along with replaced fonts and page indicators.
