iBook Author Table of Contents

I am writing in iBook using a portrait template, "Biography". I'm tearing my hair out trying to get the TOC to include the Chapters. I've watched instructional videos on Screencasts.Online and other sites, but none of them tell you how to add content that is not listed by default. The chapters & content are not being generated automatically into the TOC. Chapters 1-4 show up fine in the page thumbnails and book outline. When I try to add Sections with the + sign, I am not given any choice beyond the few that are already listed by default. These are (see screen shot attached):

Has anyone else run into this? I am doing this book as a contract and on a tight deadline, so getting desperate.

The TOC other than sections goes of the paragraph styles. Have you tried adding your chapter headings paragraph style to the TOC?
