My friend has an iBook that keeps freezing. This startup screen appears every time. Though I have seen it before, it never reoccurred for me. Any ideas what is causing this.

That's a kernel panic in OS X, it's like the "blue screen of death" from Windows.

I got a couple of those, once from installing bad RAM and another from installing beta software for old webcams.

Did your friend install any hardware or software right before this started?
 
Look in Library>Logs>panic.log. You'll likely see that the problem is the AirPort card. I've gone through this with the G4 iBook I gave my daughter. Turn off AirPort and the problem will go away.
