Can iCal be setup to have the little red circle (for notification) like Mail does for letting you know how many emails you have unread?

Not to my knowledge. However it has the alerts, which will send a tone or vibrate your phone either at the time of the calendar appointment or at an interval you choose. It also will pop up a notification.
