Icon in top left corner not going away

I have no idea what this is, but the icon will not go away. It seems to be trying to open a file, but that file is not on the computer. Expose notices it, but Force Quit does not.

Any thoughts?

Quitting Safari doesn't work?
It looks to me like an image openend in a webpage.

I have completely restarted my computer...maybe I will empty the cache. I'll see if that works.

EDIT: did not work. This thing is driving me crazy.

It seems to be almost a link of sorts. I named a file, "Driveway 004.jpg" and put it in the downloads folder, and now, when I click the "?" in the upper left corner, it opens that said file. I really don't know what is going on.
