I'm a bit confused and therefore coming here to save myself a potential rejection, but what icon sizes are required for app approval?

While it's not a complete contradiction here two links that aren't quite consistent with one another.

http://developer.apple.com/library/ios/#qa/qa2010/qa1686.html
https://developer.apple.com/appstore/resources/submission/

I only question the first because it mentions iPhone only, and iPad only as options, so perhaps it's dated? Support for both are required now, correct?

Which sizes did you include and got your app approved? Thanks

Not sure of any requirements besides having the the main 57x57 or 72x72.
Doesn't hurt to include them all though so the app icon looks good on whatever device it's on. Even if it's only an iphone app, including the 72x72 icon will ensure it looks at home on the ipad's home screen, which users can still install it on.

I'd include the 57x57, 114x114, 72x72 and as a minimum for homescreen aesthetics. I had a recent update to an iPhone-only app approved with those. Also had the 512x512 iTunesArtwork in there, but not sure it does anything outside of ad-hoc distribution.
