I used to use Iconographer, but it always messed up 8-bit palettes, and no longer runs as of Lion.

Any recommended alternatives?

Features I need:

* True .ico format (not just a .png with an .ico ending).

* Control of transparency mask

* Control of the image at each size (16x16, 32x32, 48x48)

* Control of color bit-depth

TIA!

It goes against my grain, but I’ve now tried a bunch of online tools. None offer all the control I want, and some don’t even make decent clean edges/transparency.

But FWIW, this one (Flash-based) seems to do a pretty good job:

I haven’t yet tested in all the browsers I want to (for instance, low-bit-depth cases) but so far it seems to give good results, and is simple to use. I was able to combine 16, 32, and 48 sizes in one file.
