I recently noticed an icon to the left of the alarm icon at the top of my iPhone. It looks like a phone with dots under it.

Funny story. I knew what it was when I saw it on a display iPhone 3G at the Apple store and when I asked the "genius" why they had it on, he was perplexed. Had no idea what it was and told me he'd never seen it before.

I explained it to him as simply as I could so as not to overload his already overly-stuffed with technology brain...
