Normally I have to delete them from both my phone and watch. Right now iCloud is turned off for iMessages.
 
my watch is smashed so I can’t test, but I’m pretty sure it’s the same for all device types; if you delete messages from one device they are deleted from all.

No it doesn’t work and isn’t possible right now

Opened a case with Apple last week ! Senor advisor send it to the engineers! They are aware of this and told me that it will be fixed on a update soon. I did figure a work around for this ! Wi FI seems to be the culprit on my end! When Im home I turn off wi fi and when Im on LTE they delete from the phone and the watch as it supposed to work! I have unlimited data and good LTE in my area so it works for me! No I dont have 20 messages to delete on my watch every morning!
 
If you turn on iCloud Messages then any iMessage conversations are deleted from all devices when done on one but normal SMS conversations will still need to be deleted from devices individually. I have this set up on my iPhone 12 and AW6 and this is how it works for me.

Works exactly the same for me and is to be expected
