Do I need to remove the Verizon sim card from it or is it basically not used in this phone? Also, to take jailbreak off, I simply just reformat the phone with 5.1.1 right?

I'm honestly not sure if there's a SIM in the CDMA iPhone's (although there is a slot for them).

Otherwise, yes, restore it to 5.1.1.
 
Remove the SIM. Even if the new buyer needs it, Verizon will provide them one. I always remove my SIMs.

Yes, theres a small verizon sim card in it. Probably only used for international travel, but when I remove it, my iPhone tells me there is no SIM installed.

You can remove the SIM and the phone will still work. Try it. Remove it and let it sit for a few minutes. Should work fine if your in the US.
 
Yeah, wasn't sure because I don't think my Sprint 4S has one. I've never checked, unless it's preinstalled by Sprint/Apple.
