I have a late 2012 21.5 iMac. I have istat menus 4 and it is telling me that my fan is spinning between 2500 and 2800 rpm witch I can hear. It is spinning this loud all the time never slowing down. I have uninstalled and reinstalled. I ran apple hardware test and got a 4mot /4/ 40000003: main-2730 error.
 
What are the reported temperatures?
Also, 2500-2800 r.p.m. is a fairly low speed for fans.

You may want to open up Activity Monitor, and check if there are programs running perhaps in the background using your CPU.
Have you installed anything "strange" lately?

I'd take it to an Apple Store, based on the failed hardware test (and the fact that it's not a year old).
 
I restated my iMac again and the fan was back to normal the temps were around 106 F when it was spinning that fast the normal rpm is 1400. I have not installed anything lately but I have a antivirus program from the mac app store so I run it and see if it finds anything. If it start going crazy again I will just take it back to the apple store.
 
i got some around 1800 rpm when i play some starcraft 2, and meanwhile im using for the first time time machine ,so backing up wireless (i suppose this count too increase the rpm of the fan). And yes from 1800rpm i hear the fan too
