For the 12 Pro Max, I joined the annual upgrade program. What happens if I want to switch phones and leave Apple? Can I sell the phone, do I turn it in, do I have to pay it 100% off? I guess I didn't look into those details.

In the UK at least it’s not a lease. You take out a personal loan for the cost of the phone + AppleCare. You can do whatever you want with the phone but you still need to make the monthly payment or contact the loan provider and pay off the loan. I don’t believe you can just return the phone apart from the upgrade option (which starts a new loan) after 11 months

It's a loan in the US as well. I think that you can pay it off in full after making the initial 6 payments....but there's no way to do it online, you have to call Citizen's to arrange the payment.
