3Gs, not jailbroken

Since updating to iOS 4.3 it will only plat the default ringtone and text tone set up in the settings. Any contacts that I put a different tone in still plays the default tone.

If you are sure you are assigning the tones correctly, try a hard reset, restore from backup, and then a restore as new.

Make sure you have the area code stored for the contact.

The safest bet is to delete that contact's phone number (but not the contact), have that person call you, then choose Assign to Existing Contact or words to that effect. That ensures the number is stored the way the carrier transmits it. This is the most frequent issue for our customers.
