In the keynote video at 1:16:45 in the bottom right corner "LED flash for incoming calls and alerts" is listed as new feature.
Is this for iPhone 5 only or also for the older models ?

Well the iPhone 5 does not exist at this point, and the iPhone 4 is the only iPhone with a flash- so it will only be on the iPhone 4 (and it's succeeder, when it comes out).

I don't think that they will use the camera flash for it.

Well since the iPhone 4 has no LED light (Not counting the flash on the back) ... This could be potential evidence of a new design for the iPhone 5.

Accessibility
iOS 5 comes with innovative new features that make it easier for people with mobility, hearing, vision, and cognitive disabilities to get the most from their iOS devices. iOS 5 works with specialized hardware accessories. The LED flash and custom vibration settings let you see and feel when someone’s calling. And improvements to VoiceOver include custom element labeling. These are just a few of the ways iOS 5 continues making advances in the field of accessibility.

Regardless of how they implement it (separate LED or camera flash) this is a HUGE feature! Good find!

It is not a feature for the iPhone 4. I have a developers license and installed the iOS 5 beta. I've toggled with the feature and tried multiple alerts (texts, calls, etc.) and the LED flash does absolutely nothing. It must be for a new model of iPhone that incorporates an LED light specifically for notifications.

Correct, this feature is currently only meant for the hearing impaired, it is a visual notification that their phone is ringing.
