Has anyone had their iPad turn off, or go to sleep rather, in the middle of using it?
 
Only when I had a case with a crappy smart cover. When the over was folded all the way back it would trigger the iPad to turn off.
 
Never had this happen with any of my iPads. I would call Apple and make an appointment to get yours looked at if this is what it is doing for you.

When it's running out of juice, an iPad should go to sleep in a particular way. The currently opened app should quit itself and the launcher should vanish, before you get a progress wheel on a black screen.

If everything is just going black suddenly then you've got a hardware problem.
