On my iPad and iPhone I keep getting a annoying reminder but always decline it. Though it always comes back :(

How do I remove it for good ?

It like a advertisement.
 
I keep receiving such reminder each time I restart my device but that because I Have a reminder which I have zero intention to even mark it as complete, yet it is set to repeat everyday, indefinitely.
Check your reminder list and see if there is anything unusual.
 
I already declined the offer/reminder.

Anyway it is asking me, if I remember right, to sign up for something.

Seems like you subscribed to a shared reminder list at some point.

Tap the "edit" that's in the lower middle part of your screenshot. That should give you the option to delete that reminder list.

If that doesn't work, call Apple and report that user (whose email is showing in your screenshot) as using the Reminder app to send spam.
