Both my wife's and my iPhone 12's have the same issue. Our voice connections are like finger nails on a chalk board. Apple says its on Verizons end but Im thinking the voice connections are on apples end. The mic picks up the background noise of conversations and presents it louder then the person your speaking with. Almost like while on speaker phone but your phone is at the end of a hallway with a few people speaking in between. We've both reinstalled iOS 14.1, per Apple support, and started from scratch with the same results.
 
Make sure you don't have wifi calling turned on. When I got my 12 pro, and checked cellular settings, I noticed it was on. I never turned it on, so for some reason it's turned on by default. So I turned it back off.
 
Ok. I'm on Verizon, and my calls sound clear.
Happens to both of us calling from our respective phones to different individuals. If your put on hold all is fine for but once the mic starts to pickup noise it goes crazy. Theres no way a TV in the background, while not on speaker, gets picked up clear and the voice front and center sounds heavily modulated or altered.

Ok, go to Settings, Accessibility, Audio/Visual, Phone Noise cancellation. If they are both on, turn them off. Then make a call.
 
After 2 calls to Apple and following their instructions the problem is unresolved. Apple claims carrier issue. Called and went to Verizon and Verizon claims hardware issue. Return and got my old phone back.

Smart move I know you don’t got time for the back and fourth games with apple
