Iphone 13 voice over randomly activates

Out of nowhere Random squares outline certain boxes or texts. Then locked out of phone and have to take it in to hack into it and sure enough, voice over is automatically activated and has to be disabled again and again.

Edit: For anyone that’s had this issue. Fixed by going to:
Settings / Accessibility / Accessibility Shortcut / Voice Over (Uncheck)
