iPhone 4 Audio Output Issue

Ok so this is a problem only with the internal speakers ..... hookup headphones and it works fine. Before you suggest a switch in the headphone jack please read below......

What DOES WORK with Internal Speakers .....

Ringtone
Alarm


What DOES NOT WORK with Internal Speakers
iTunes
YouTube
Netflix


I tested iTunes playing music through the headphone jack and it was fine.

Any ideas?

UPDATE:
Did a complete reinstall of IOS and the phone worked for " a little while " now again there is no speaker output for iTune, YouTube, and Netflix.
