This isn't all the time but it happens frequently...

I'll give an example

my phone is in my pocket, I heard it ring, I see it's my mom calling.. I slide to answer but it doesn't recognize my touch. The touchscreen is completely unresponsive... phone call ends.. she calls back and same thing.. no response... I push the lock button and then home then it responds

seriously though, ive heard of this problem before. i think people push the sleep button to fix it
 
I know exactly what you mean OP, its completely freezes. It happens to me once about every 30 calls.


or maybe my iphone doesnt want to answer your mom either
 
Uh latest version I guess and no jailbreak

Nope. When's the last time you cleaned out your multitasking tray? Also, try restoring to a fresh 4.3.5 or 4.2.10 (GSM vs CDMA). Might help clear any software related issues.

A restore won't help.

I have been having this issue since my first iPhone and iOS 1.0, and the problem persisted with my iPhone 3G, iPhone 3GS and now my iPhone 4.

Correct.

When a call comes in that you are unable to answer, press the Sleep/Wake button once to mute the call, then for some reason the Slide to Answer button will work once again.
