iPhone 4 - Duplicate Icons after restore?

Can anyone help me to identify a cause for this? This is the second time it's happened and can only be corrected by "resetting to factory" and restoring as new. The Phone, folder and SMS icons were moved. I had them on the dock but moved them to show the problem. Thanks in advance.

I am using the latest version of iTunes, Windows 7.
