iPhone 4, registers sim but won't call out

Hello, trying to fix this problem. T mobile iPhone 4 with signal but when I try to make a phone call or send a text I get a no service message at the top where the signal should be.

It's on 5.1

Do you have 3G off? Are you sure it's unlocked?
