If you are not in the US then online orders do not come with a micro-sim card.
However in a lot of countries you can walk into an Apple Store and get one free from whatever official carrier as long as you bring your receipt for your online order. If you don't have an Apple store close to you then your local carrier store will sell you it for a pretty good price (about $10)
