worth replacing for loose volume buttons? I hear the 'metal on metal' when I touch the volume buttons, but it doesn't rattle if I shake the phone, etc. -- only when I tap the buttons.

Never experienced this but it doesn't sound like something worth replacing.
 
If it bothers you, go ahead and replace it before your warranty expires. I replaced one of mine because loose/rattle sleep button. The genius will give you weird look though. Like a, "you serious bro?" kinda look lol. I'm very OCD about loose buttons btw.

Dear Honda there is a rattle in my car when i slam the hood, please give me a new one
 
in all honesty, the noise goes away when I have the Speck Pixelskin case on it (which I plan on keeping on there). From the horror stories I've been reading with people having to replace their phones countless times over little rattles, light leakage, etc., I think I'll take a gamble and keep this one, as it's flawless otherwise.
