I haven't yet found any definitive source on the internet showing what version of iOS shipped on the iPhone 4s. I also watched a few unboxing videos, but didn't see any that showed the about screen. Anyone know what versions of iOS shipped on the devices when they were sold new?
 
iPhone 4s was available for sale on October 14, 2011.

iOS 5.0.1 was released November 10, 2011.
 
Always the x.0 for iPhones upon launch. If there's a severe bug, there might be an update week of release.

Almost one month after isn't around the same time. Besides, later batches of iOS devices tend to have latest available firmware at time of manufacturing installed. An iPhone 4S purchased at launch will likely have older firmware than one purchased months after (unless the latter is just old stock).

After some research, here is the answer: If the serial number is known, then http://sndeep.info/en can be used to look-up the manufacture date. From there, research the iOS release dates to take a highly educated guess regarding what iOS shipped on the phone. For example, if the manufacture date was the year 2012 and its current age is 7 years and 2 months, then we can surmise that the iPhone 4s shipped with iOS 6, because today is 12/24/2019 and iOS 6 was released on September 19, 2012. [The above-mentioned website even shows the week of manufacture; i.e., week #40.]
