I have seen it happen a few times on my iphone has anyone else experienced it and how did you get it?

I saw it in iOS 6.0 but I think it was fixed in the latest update (or should have been) 6.0.1.

First, are you on iOS 6.0 or 6.0.1? A lot of people experienced it on 6.0 but it was supposed to be fixed on 6.0.1. I know mine was at least.
