The iPhone 5 uses a nano-SIM, not a micro-SIM, and yes, it's included.

If you ordered your phone from ATT, or from Apple for specific carrier and phone number, it will come with sim installed and all ready to go for that number. All you need to do is activate it (follow instructions with phone) and it will work. If it is an upgrade then once it is activated your old phone with the same number will stop working.

Yes it will come with everything that is needed to activate it. They do not care to see a million people coming into their stores on Friday.
