Iphone 6+ battery issue

My 2yr 6+ started acting weird. Once every couple of weeks the battery will go down from 60% to 1% then turn off in the matter of 1 min. Once i plug it into the charger it goes back to 60%+ in a few min. According to Battery Life app my battery reads 17.7% Battery Wear Level with 2400/2915 mAh. Any Ideas ? TIA
