iPhone Account Help- ATT, Please Help (Activating)

How do I reactivate? I think iTunes thinks I'm already set when I'm really not because I'm using my existing SIM that went from my iPhone, to a RAZR(removed iPhone from plan), now back to the iPhone...

iPhone says its already activated but when I go to voicemail, visual voicemail is not there.

Please help- Thank you!
