iPhone battery percentage going down while plugged in

As the title says my iPhones battery has been slowly depleting while plugged into the wall. Any ideas?

It happens and I would even say it's normal. Unplug it and plug it back in.

same here.

On a road trip yesterday, plugged into 12v car charger, streaming Pandora and using GPS and I saw it slowly regressing!

Killed Pandora, still going down using GPS only.

Today on return trip, same setup and it was fine...charging while running both apps. go figure.
