You have to write down the 15 digit full IMEI number to know what carrier of your phone is it. OR you can contact to your network provider to know more on it.

It's never advisable to post your IMEI on the Internet, so please don't tell this guy to do so. Nor do I think the full IMEI would be needed to determine the carrier info.
OP, just take it to an apple store. They should be able to tell you what carrier it came from.
