Is there a disadvantage to getting an iPhone refurbished for a cheaper price? What is the most serious thing that can be wrong with it?

check warranty and if it was refurbed by Apple it's the same specs and conditions as new as older models are all refurbs Apple is not going to continue to build older models. Some new models have issues out of the box as refurbs. all depends on what you want to pay.

As long as its refurbished by Apple there's very little downside.
They replace the battery, check all functionality, and give it the same warranty as a new one.
It's the refurbs from 3rd parties that you need to be aware of.
I wouldn't trust any refurb bought from eBay or Craigslist or from any warranty provider like Square Trade.

Yeah. Can a refurbished iPhone from Apple have any major scratches or nicks?

Nope...you won't be able to tell the difference between a refurb and a brand new one. Apple's refurbs are actually BETTER then new ones because they get thoroughly check by Apple before being used. If there's a nick on the metal band, or any other part, it gets a new one before its boxed up. It's basically a brand new device.
