I added a custom ringtone, and it's fairly low so I'm having to put the ringtone volume to 100%. I've never done this on any other phone and for some reason the iPhone seems to have lower ringtone volume output than any other phone i've owned. Is this a defect or has anybody else noticed this as well?

Well, a music file has an inbuilt volume of recording. You can use programs like Audacity to modify the file so that it has a higher volume. Give that a try.
 
i tried to boost the output (db?), but since auto normalization basically exports every song at the maximum loudness before it starts distorting, even an additional +0.4db started distorting the bass, etc.
