i guess another way of asking is, has anyone traded in a phone with visible surface scratches and still received full "max" credit for their phone?
I don't know how picky the carrier is when it comes to visible surface scratches, I know Apple checks and looks at everything, they even perform diagnostic testing

carrier 'offers' (where they provide bill credits over many months for some $ rather higher than the actual trade-in value) - in an Apple Store - doesn't matter so long as value isn't zero.

I don't know about direct carrier trade-in

Are we talking normal wear and tear or something more extreme? If the scratches don't effect the usability of the device, your probably fine

yes, just normal wear and tear, super minor scratches.
