2 Macbooks in the house (M4pro/M2Air). 2 iPhone 16 Pro Max on different iCloud accounts. When on a call and if you hit the "audio" button, you get options for iPhone/speaker/and 1 of the MacBooks. The problem is on my iPhone it shows the MacBook Air, when I have the MBP on my iCloud account. VisaVersa on the other iPhone, it shows my MacBook Pro as an option for an iCloud account that has never been connected to the MBP. I have tried turning off Airplay handshake and continuity camera on both iPhones. Did't change anything. When you actually choose one of the Macs, you don't actually hear any sound from the iPhones. Any ideas? Bug?

Check your settings on each phone. Things like wi-fi calling for other devices or allow calls on my other devices might give you a clue.
