With the necessity of it fitting properly, would you advise against buying one online where you can't test it out to make sure the fit is good and everything?

There are plenty of third party retailers or an Apple Store you can try the Watch on before purchasing. If you have not tried the Watch on before, I would not suggest blind buying for size wise online to determine if the 38 MM or 42 MM is more appropriate.
 
I went to the Apple Store to try one own and check out the different sizes, bands etc.. I then bought a mint used one on eBay that had AppleCare and saved a lot of moola. I do recommend laying hands on one before buying it. I initially thought the smaller one was the right one for me, but after trying them on went with the larger size.
