2 quick questions:

1) is a factory unlocked iPhone 6+ always unlocked?

2) Without having a 2nd sim card, what's the easiest way to determine if the phone is in fact unlocked?
 
Where did you buy it for what network?

Did you pay for it to be unlocked or are you assuming that its unlocked?

I bought it from someone who sold it to me (last year) as a factory unlocked phone. While I was sitting with him, I took out HIS sim card, inserted mine (verizon) and made a call successfully. Put his sim card (ATT) back in and successfully made a phone call, indicating that it was unlocked.

I was just wondering this morning if it is still unlocked or does the phone eventually lock? How can I confirm?

If its an ATT phone and you can use the VZW network, I think that qualifies as being unlocked.

Once is unlocked it can't be locked again :)
 
I feel confident that it is unlocked, but I was just wondering if there is a way to go into the settings or some other way besides swapping sim cards to verify for sure that it is unlocked, just for future reference.

Is there some place in the settings that indicates that it is in fact unlocked?

No, there isn't anyway other than inserting a different sim card into it.
 
No, there is no where in settings that will tell you whether your phone is locked or not. You will just have to insert a different carrier sim to find out.

Since you mentioned that you tested it out by inserting your verizon sim into the att phone and it worked, so therefore it is permanently unlocked.

From what you posted, it's unlocked.

If you want to re-verify without a sim, you could always restore in iTunes and at the end of the restore you'll receive a "Congratulations, your iPhone has been unlocked" message.
