Ideally, I'd rather not give some 3rd party app permission to use the camera. I'd trust Apple to scan a QR code but I'm darned if I can figure out how to do this seemingly obvious thing :-(. Any clues would be most welcome!

I tried it myself recently with PhotoBooth, it didn't work automatically, so I gave up and used the phone. It appears there are apps that add on this ability to Mac, so I guess it's not possible OOTB.

What would happen if a QR code leads to a website (and click?) you would never dare to go to? Just a thought.
My FireFox browser 'knows' about all currently registered criminal sites, but my Mac .....
;JOOP!

I agree that a QR Code could take you to a sketchy website - that's certainly a risk. However, I wouldn't want to compound that risk by giving a 3rd party access to my camera on the Mac. Apple already cater for this in iOS, so why not on macOS? I've already filed my feedback to apple.com/blackhole - err, I mean, apple.com/feedback.

Easy work around when reading QR-code that leads to www-site is to just read qr-code with your iPhones camera and click the link to get to www-site. Copy the address from your www-browser in iPhone and paste it in your MacBooks browser, and you are there.

You of course should have Apples Universal Clipboard in use. If not you could paste the url fir example to notes in iPhone and pick it up from there in MacBook.
