I got this email from my boss today, whos not great at technology type stuff, this is what it says:

"I am trying to figure out how to use my Ipad as an address sign in sheet at my upcoming shows. Does any one have any idea how I can do this so that I can get addresses into a data base that is easy and customers can do with out my help, directly on the Ipad. I would like their name, address, and e-mail address, phone #."


SO basically we are looking for an app that her customers can input their info into and it will kind of keep a data base of their info so she can use it later.

Bento comes to mind. Of course if the info would need to be exported, it would require a Mac and the Bento software for Mac.

You could set up a spreadsheet in either Numbers or Docs2Go.
