I have a client who is really into marketing. He has asked me to create a squeeze page that involves a customer having to enter in their details into a form, and then that form inputs that info onto a coupon for them to print, and then the info gets sent to him.

Has anyone seen anything like this before, or know of software that offers this sort of service that I could embed into a website?
