Itunes 11.3 won't shuffle

Just started doing this. When you hit the shuffle button, the same song (the one that starts playing) is populated in the Up Next que.
Doesn't matter if its a smart playlist or a genius list, it won't shuffle and re-plays the same song over and over. Any ideas?
