iTunes App: Rating a song 5 stars will no longer put it in the "Top Rated" playlist.

I was wondering if anyone could help me out with this. I always have my top rated list on iTunes as a way to organize the songs with 4 or more stars. I used to be able to rate a song 4 stars and it would automatically place that song inside that play list; it no longer does it.

I have tried deleting entire iTunes files and re-importing everything manually; no luck. I have tried from a different company; still nothing.

Is this happening to you? How can I solve this issue?
