Here's what you should to when the code of your iTunes card is not accepted by Apple:
1. If you are entering the code within iTunes, stop!
Do NOT proceed to the next page as show in the attached screen shot. Why? This option does not allow you to upload additional information which are required. You will end up delaying the process.
If you are entering the code on iPod, iPad and iPhone, you'll need to get to a computer.
2. Use a computer and visit the following page, and request support via Express Lane.

- iTunes Card code is not activated
(fill out all required information)
3. Finally, the most important part is, take photos of (a) sale receipt, (b) front and (c) back of the iTunes card. Make sure the photos capture the ENTIRE card surface, and all the fonts are readable.
Attach these images as you finish up with the submission of support request. They have a limit of 15GB of all photos, that should be plenty.
If you fail to upload these pictures, the support rep will email you asking for them, which will delay the process.
In my experience, it took 48 hours for the rep to reply your first contact. When subsequent follow-up was needed they will respond in 24 hours. They do work 7 days a week.
