I have been having this issue for awhile and keep hoping that the next iTunes update will fix it, but I keep waiting....the newest update still leaves me hanging.
I do not use iCloud or Music. I just buy songs/albums (99% of the time directly from my iMac) and then sync them to my phone. I get the wacky duplicate playlists issue, which is annoying, but easily fixable.
However, I also have an issue that if I have not listened to a song on my computer, it will not sync to the playlist on my phone. The songs actually download into All Music and the appropriate album, but will not show up in my playlist until I listen to it on my computer. This is very cumbersome as I typically just listen to my music in my car, so I can't buy an album, sync, and then go. I have to listen through every song once before it shows up on my playlist.
Also with this problem, is that if I look through iTunes on my computer at the device and the content there, it actually shows the songs in the playlist. My computer thinks the songs are on my phone in the right playlist when they actually are not.
