How do I get my phone to have custom Icon layouts like 5 in the dock and how do I get cydia to stay in a folder on 4.0.1? Everytime I restart my
Phone it leaves it's folder. Thanks

Install the "5 Icon Dock" tweak if you want that specifically, otherwise just have a browse through the tweaks in Cydia.

As for it leaving the folder I have no idea, mine sits in it's folder no matter how many times I've respiring or done a reset, and that's running on both my iPhone 4 and iPhone 3G.
