...and installed the SoftRAID beta on my M1 Mac mini, and the preconfigured RAID 5 volume mounts and shows up as hoped... but then I go to look at it and see the following in the SoftRAID info - take a look at disk5. Doesn't seem to be a new drive, has a previous i/o error? What do you think I should do? Contact OWC and ask them about it? Ask them to send me a new drive?

If you bought this from them with the drives installed then yes, I would contact them about it. No way should a new drive have over 700 hours of use on it when the other drives show 39 hours of use. Looks like someone grabbed a used drive instead of a new one to install in the enclosure.
 
Talked first to sales support, who were not helpful, they just said they don’t ship used drives. I said of course you don’t ship used drives on purpose, can I please talk to tech support instead? Tech support was way more helpful, and in the end they are shipping me a replacement drive and sent me shipping label to send back the weird one. OWC is a good company.
