With the iPhone 5. The picture doesn't fill the whole screen? Is there a setting to change this? Or it's working as intended?
 
That's "as intended" given the resolution and aspect ratio of the screen being 16:9. As you zoom in the picture will fill the wide screen.
 
Quick question on this. When you are looking at the screen prior to taking the pic does it fill the whole screen? With the 4s, what shows on the screen is what comes back as the picture. Does the 5 show the black bars prior to taking the picture?

The interface of the camera app covers more of the screen than before, leaving the area left for photo viewing about the same shape as the photo that gets taken:
