The app I dislike the most on macOS is Keychain Access. I don't want to go into the details of why I dislike it, I'm just looking for third party apps (like 1Password, which I already use) to take on most if not all of what keychains are handling. Is there any way to throttle down what the OS passes to and retrieves from the keychain (certificates, keys, etc) and have those handled by a third party app, one with perhaps a better user interface? I'm on both Sonoma and Sequoia at present.

Certificates need to be in the Keychain for apps and the system to use them. There's no workaround for that.
