Has anyone found a reliable Lightning cable extender for adapting Lightning equipped docks for use with cased iPhones?

And why are a lot of the adapters designed with this weird shelf on the female end?

Why not just a flat surface to mate with the existing dock?

How about getting this dock? It works with virtually cases since the placement of the lightning connector and back support are all customizable.

It's designed to work with a LifeProof case. The extender is sized to fit into the hole in the case, then the shelf on the extender allows the door on the case to snap in, holding the adapter in place.

How about getting this dock?

Nice but not what I asked for.
I want an adapter to convert existing docks like those from iHome and other Lightning-equipped audio accessories.
